School and camp forms are provided at annual visits. These forms contain information (immunization history, medications, pertinent medical conditions, and a statement of health) that schools and camps require.

This form is sufficient for school, sports, and camp activities for twelve months from the exam date. Make multiple copies of this form for use during the year. Copies of the most recent school form can be downloaded from the “letters” tab in MyChart.
